Surgical Consultation and Surgery Appointments

To make an appointment for surgical services with Drs. Rappaport, Kip, Olson, and their extenders: Stacy Mierau, PAC; Shannon Rumble APN, And Rachael Hueftle APN, please contact Jenny at 775 448 9414.

Please note that during you scheduling call, important information will need to be gathered by the scheduler.  Please provide the following information to the scheduler when asked:

1. Your complete name and correct spelling.
2. Your mailing address.
3. Your telephone number for home and work if applicable.
4. Your marital status.
5. Your date of birth.
6. Your insurance information (your primary and secondary insurance if applicable).
7. Any prior testing, such as MRI or x-ray.
8. The name of referring physician, if applicable.
